Update as of Dec 18, 2012:
Starting Jan 1, 2013, ang monthly Philhealth premium for Voluntary or Individually Paying and OFW members ay 200 pesos na.

For individually paying professionals earning more than 25k a month, ang monthly premium ay 300 pesos (as announced in 2010 below).

For voluntary membership, just bring:

  1. your IDs
  2. 300 pesos to pay one quarter (100 pesos per month), which is the minimum payment. You can pay 6 months, 9 months, one year or more.


Effective October 1, 2010, Philhealth premiums for professionals with a family income of more than 25,000 a month will increase to 600 pesos per quarter, or 2,400 per year, and then to 900 pesos per quarter or 3,600 pesos per year in subsequent years.  See Philhealth Premiums for Professionals.

If you have dependents, bring original and xerox copies of:

  1. birth certificate of your unmarried child younger than 21 years
  2. marriage certificate, if your spouse is not a Philhealth member (Bring a certificate from your spouse that he/she is no longer active if he/she was a former member.)
  3. birth certificate of your parents 60 years old and above
  4. your birth certificate if you’re filing your parents as dependents
  5. court decree of  adoption for a legally adopted child

Membership is still 100 pesos per month (or 200 pesos per month for professionals), whether you have dependents or not.

  4. Hi Angelo, you can register online at and you will receive a Philhealth no. If the online is registration is still temporarily off, visit the nearest Philhealth with your ID and register. Kaya nga lang pag sa branch, they require you to pay the minimum 3 monthly contributions, equal to 600 pesos.

  6. Hi Apple, based sa mga nagco-comment dito, hindi pa yata naayos o na-restore ang online registration nila, kasi noon, agad-agad na naibibigay thru email ang Philhealth no.

  8. Hi Antonette, ang premium for Individual Payor-Informal Sector ay 200 pesos per month. Minimum 3 months or 600 pesos kapag sa Philhealth ka magbayad.


  10. Hi Metchel, you can verify your Philhealth no. at the nearest branch. Kapag many years na nag nakalipas, mag-register ka na lang uli either sa branch or online.

  13. Hi virgilio, yes, you can continue as Individual Payor. Use the same Philhealth no. You can pay at Bayad Center, or if you like to have your updated MDR, go to Philhealth with your ID and update your MDR (Member Registration Data updating) so you can get your updated MDR, and pay there also.

  15. Hi Ma. francisca, yes, you can continue kahit wala ka nang any Philhealth paper. Just go to the nearest Philhealth with your ID and fill up the registration form. Check FOR UPDATING. Leave blank your Philhealth no. because you no longer know it. It will be Philhealth that will write your Philhealth no. Then pay for Oct to Dec or more quarters in advance, if you like.
