Here’s a list of documents that you need to submit when filing your SSS maternity benefit claim for miscarriage:
FOR COMPLETE MISCARRIAGE:
(Raspa or D&C was not performed)
1. Maternity Benefit Application (if Voluntary, Self-employed, OFW or Separated)
Maternity Benefit Reimbursement Application (if Employed)
2. Maternity Notification
(stamped by SSS before date of miscarriage)
Note: If you were not able to file your Maternity Notification before miscarriage, fill up a Maternity Notification form, and then attach a letter to SSS explaining why your Maternity Notification was not filed before miscarriage.
3. SSS ID card
(or your SSS ID application acknowledgment stub
and 2 valid IDs with signature and at least 1 ID
with photo)
4. Obstetrical History indicating number of pregnancies and
Hydatidiform Mole (H-mole), certified by attending doctor,
with PRC License No., printed name and signature
5. Pregnancy Test result before and after miscarriage
or Ultrasound Report before and after miscarriage
6. Medical Certificate, indicating circumstances of miscarriage, with explanation of why D and C or Raspa was not performed, issued by attending doctor
Now 105 Days!
FOR INCOMPLETE MISCARRIAGE:
(Naraspa or D&C was performed)
1. Maternity Benefit Application (if Voluntary, OFW, Self-Employed, or Separated)
Maternity Benefit Reimbursement Application (if Employed)
2. Maternity Notification
(stamped by SSS before date of miscarriage)
Note: If you were not able to file your Maternity Notification before miscarriage, fill up a Maternity Notification form and attach a letter, addressed to SSS, explaining why you were not able to file your notification before miscarriage.
3. SSS ID card
( or your SSS ID application acknowledgment stub
and 2 valid IDs with signature and at least 1 ID
with photo)
4. Obstetrical History indicating number of pregnancies and
Hydatidiform Mole (H-Mole), certified by attending doctor,
with PRC License No., printed name and signature
5. Dilatation & Curettage (D&C) Report,
certified by authorized hospital representative
6. Histopathology Report, certified by authorized hospital representative
7. Pregnancy Ultrasound Reports or Pregnancy Test results
before and after D&C
8. SSS might require additional documents if basic documents are defective or
not valid
FOR SEPARATED, PREVIOUSLY EMPLOYED, SELF-EMPLOYED, or VOLUNTARY MEMBERS:
If your miscarriage occurred while employed or within 6 months of your date of separation from your company, submit these additional documents:
. Certificate of Separation from Employment,
with effective date of separation
. Certification of Non-Advancement of Maternity Benefit
(certification from employer that no advance payment was granted to you)
. Two sheets of L501 Form from your former Employer
– The signatory in all these 3 documents should be the same person.
– The signatures in all 3 forms should be the same.
– When you go to your former company to ask for L501, bring 2 sheets of L501 form in case your former employer does not have this form. You can get this from SSS or you can download from sss.gov.ph
NOTE:
Remember that the maternity benefit is a payment for the no. of days you missed work due to child delivery or miscarriage, so you should take your maternity leave, as this is the type of leave being paid by the SSS maternity benefit.
Make sure also that you’re eligible for the maternity benefit based on the basic requirements, such as no. of contributions within the 12-month period before your semester of delivery.
For Those Who Went AWOL, or Have a Labor Complaint Against Their Former Company, or Have Strained Relations with Their Former Employer,
you can use this Affidavit of Undertaking form, issued by SSS.
Here’s helpful information from Michelle who filed her SSS Maternity Claim for Miscarriage in May 2016:
I had filed my Maternity Claim for MISCARRIAGE (Incomplete Abortion with D&C Procedure) this May 30,2016. I wasn’t able to talk to the doctor (cause there’s none on duty), so I was told to go back on June 1 for approval.
I just got my claim and was APPROVED already (yeey!) and luckily my company would reimburse me right away this coming pay-out. Here is the list of the documents I’d submitted. Hope this will be of great help.
- MAT2
- Med certificate (Orig copy)
- Complete Obstetrical History (form available at SSS or your HR Dept)
- Clinical Case summary/medical abstract (Certified true copy)
- Discharge summary (certified true copy)
- Surgical Memorandum (certified true copy)
- Histopathogy Report (certified true copy)
- Mat 1
- Ultrasound result before my miscarriage (certified true copy)
(I didn’t have ultrasound result after since I have other supporting docs)
- Photocopy of valid IDs
- Anesthesia Record (I guess this was just a supporting document but will be given still by the hospital)
- Our HR dept also provided me with my cert of contributions to SSS for the past 6 months (but I guess this is just a supporting doc also)
P.S.
Make sure to have the hispathology report to be certified true copy because this was a big issue with my colleague’s application. Since it was done by another lab, the hospital refused to certify it and SSS wants it certified.
If your hospital is also conducting the same test, better do it there if the price is reasonable. I got mine at P1,200 but the price range outside is from 900-2500 depending on the laboratory.
And lastly, make sure the date of your procedure would be the same in all your documents to avoid inconvenience at SSS.
I hope this will help because before I was confused with the list of requirements given by SSS as what were given at the Record Section because they were labeled differently.
